Membership Cancellation Request

We’re sorry to see you go, but we understand that circumstances change. If you decide to return in the future, we’ll be here to welcome you back!

Important Details:

• If you choose to reactivate your membership later, the current membership rates at that time will apply, as rates are subject to change.
• If you’d prefer to pause instead of canceling, you may submit a Membership Hold Request.
• To proceed with cancellation, simply complete and submit the form below. This will serve as your 30-day written cancellation notice as required by your membership agreement.
• Your membership will be canceled 30 days from the submission date of this form.
• If a payment is scheduled within this 30-day period, it will still be processed as scheduled. All payments are non-refundable.
• Cancellations made before the contract expiration date require a fixed buyout fee of $250.
